I think perhaps the high heel stigma can be updated along with a few other things you see in the standard porno. I think the entire industry should examine the quality of their films and make the necessary adjustments

You answered your own question, MS, . . . . it's not supposed to be anything 'realistically'. It is for the sake of the fantasy.
For some watchers it may be the fantasy of taking a 'naughty' or even a 'slutty' woman into bed with you. For some there may be the danger aspect--watching something that appears more risky can be more exciting! The movie is not saying one should actually wear the heels to bed, necessarily, but that you should enjoy looking at the girl in the heels, indulge the voyeur in you, and just enjoy the view.
